This form is not intended to replace the normal procedures, paperwork or tags used by the Ground Safety Committee. It is simply another tool to bring attention to safety issues in your workplace that need to be addressed.
Remember if in doubt always file an ASAP!
After submission a member of the Ground Safety team will contact you for more information and to follow up on your concern.
